It is possible to get topless images by 'accident'. What you have described does seem strange though. I have gotten topless images from totally legitimate prompts, from this Reddit. Sometimes it is the context; in the last topless i received it was a reference to horror - the AI extrapolated that horror means topless! It only happened once from 5 o6 6 times using the prompt.
